All creatures in Silent Hill 2 except Pyramid Head are female since they are James' projections of Mary. As an exception, Abstract Daddy is a creature being projected by Angela, which James & we are looking at by his perspective. x.com/adsk4/status/1…
Abstract Daddy which James was looking at was little different from Angela's one. And James was seeing abuse about her through that creature abstractly. I leave the image of Angela's one to player's imagination.

The mouth of Flesh Lip & Mandarin's face symbolize the mouth of Mary verbally abusing James. They were inspired by the mouth of the figure in the center painting of “Second Version of Triptych 1944” by Francis Bacon. As a side note, Mandarin appears only in Otherworld in the SH2R
