Ambulances can’t get to dying people because they’re stuck outside of hospitals full of patients we could discharge if we just had more carers in the community, so I’m delighted that the government has taken decisive action to ban these carers from living with their families.
Today in Parliament we have laid an order to ban overseas care workers from bringing dependants. This is just one part of our plan to deliver the biggest-ever cut in migration 👇
Hate that critical incidents need to be declared, but respect organisations that recognise the shit they are in & take proper steps to address issues threatening patient safety. We had more patients waiting for longer in York this week @YSTeachingNHS yet no incident declared.

Those specialties have the option of pushing back to the ED. We have nowhere to push back to, other than the ambulance ramp, or the waiting room. For too long, Trusts have become far too comfortable with the disproportionate concentration of risk in the ED #crowdingkills
The ABC of Emergency Medicine. A is for Aorta You should always think aorta. Aortic dissection can present atypically. It can present painlessly. It can present 6 months after you've seen the patient. Probably safer just to scan everyone.

Re: cohorting If trusts can make space for pre-ED cohorting to enable ambulance handover, they can make space for post-ED cohorting. Often the reason this doesn't happen is it's easier to ask EM to carry the can than specialties. This approach ignores the root causes of crowding.
