Let’s agree that DAPT given after mild stroke or high-risk TIA due to athero should be limited to 21 days. Most of the benefit in terms of recurrent stroke risk reduction occurs in this time frame, and thereafter the bleeding risk continues to rise neurology.org/doi/10.1212/WN…
I love it! Hope this becomes a universal practice! Unfortunately some hospitals are super restrictive about it for fear of infection (?) and staff safety. Here is an example from last year when I was visiting a very reputed hospital in India!

We find all sorts of incidental finding in our trauma survey scans. PEs are definitely an incidental finding that complicates decision making. I for one think we are probably too aggressive with our anticoagulation strategies…
