The CHW-led diabetes program is up and running! At St. Vincent's/Coastal Health, we are nearing 100 patients in the program thanks to GIM's Dr. Vaughan (PI), Dr. Farr (medical director), Drs. Wilkerson & Ahmed (Physician Champions), and Family Medicine & Wellness leadership.
