I've generally found that the best product ideas live at the intersection of "duh" and "holy shit".
Are there any successful examples of companies started in the last decade or two that have nearly no product managers? Perhaps mostly design and engineering collaborated. Would love to learn how it worked.

A friend who is an MD specializing in infectious diseases was told at work today that the US is about 10 days behind Italy.

A company's focus areas often look like a barbell. Pre-seed: go wide; experiment w/lots of use cases & customer types. Seed/early Series A: go narrow; nail one use case for one customer segment. Later stage: go wide; begin supporting additional use cases and customer segments.
