Improving quality of later life sometimes means less medical intervention not more. This should be the patient's informed decision. Older people and their families need to have realistic conversations with their doctors including about levels of intervention they want. (5/5)

As we head into winter, with hospitals already in crisis, one of the kindest things we can all do is to identify our patients who might benefit from advance care planning. 🧵 incoming… 1/n

12 hour shift on 🚑 today. 2 calls have been EoL. But no provision has been put in place. So 2 elderly patients have had to be taken from their bed and transported to 🏥 because no1 has been willing to talk about End of Life care. Please. Have the talk. It makes it easier.
