How many inquests lead to PFDs which touch on issues where lessons should have been learned from previous inquests and previous PFDs? Answer: too many - there is a very good argument for a National Oversight Mechanism.

Today Green MP @carla_denyer will present a Private Members Bill to Parliament calling for the establishment of a National Oversight Mechanism. This is an important opportunity to further calls for independent oversight of recommendations following state-related deaths.
