Be aware if there is progression by CT which means the immunotherapy response is declining.

If laparoscopy is performed, multiple biopsies from all of the abdominopelvic regions should be performed. The only indicators of response would be tumor markers and multiple biopsies. No important imaging features for response.

Radiologic assessement of response to immunotherapy is challenging. This is especially true for peritoneal disease where multiple sites of the abdomen is involved. Frequently , we do not see a "shrinkage" of the masses but there is still a high likelihood of pathologic response

Considering prolonged stability of disease on immunotherapy, would evaluate for CRS HIPEC at this point. She does not have absolute indicators of unresectable dz by CT scan. Diagnostic laparoscopy would be reasonable to assess small bowel considering high volume of disease

The CT scan appearance is interesting because, although it shows a high volume peritoneal involvement, the hipodense, mucinous nature of the deposits and the distribution of the disease is very reminiscent of PMP and not so typical as CRC case

CT suggests pseudomyxoma peritonei of the ileocecal valve region. May also be high-grade appendiceal mucinous tumor. The MSI status should be carefully noted.
