46-year-old man is found to have 32 colonic polyps and 3 small gastric polyps on endoscopy. H&E of a colonic and gastric polyp are shown. Genetic testing revealed heterozygote pathogenic MUTYH mutations on exon 7 & exon 12. Full case can be found at: ipa-na.org/COM/03-2023
